MAIB Investigation report: ERRV Viking Islay
MAIB investigation report on the investigation of the work undertaken in a dangerous enclosed/confined space and the consequent attempted rescue on board ERRV Viking Islay, resulting in the loss of three lives at the Amethyst gas field, 25 miles off the East Yorkshire coast on 23 September 2007.
