Accident Reference

Severity of Casualty
The following definitions should be applied
  • FATAL - Where death occurs in less than 30 days as a result of the collision and does not include death from natural causes or suicide
  • SERIOUS - Examples of a serious injury include:
    Fracture, Burns (excluding friction burns), Internal injury, Concussion, Severe cuts, Crushing, Severe general shock requiring hospital treatment, Detention in hospital as an in-patient, either immediately or later, Injuries to casualties who die 30 or more days after the accident from injuries sustained in that accident.
  • SLIGHT - Examples of a slight injury include:
    Sprains (not necessarily requiring medical treatment), Neck whiplash injury, Bruises, Slight cuts, Slight shock requiring roadside attention. (Persons who are merely shaken and who have no other injury should not be included unless they receive or appear to need medical treatment).

    Always use the 24 hour clock.
    Code midnight as 0001.

    The information is entered in two parts, Road Class and then Road Number
    e.g. A127 or B1066
    If collision is at a junction give the road with priority (right of way).
    If roads have equal right of way give A road before B road etc.
    If roads are equal class e.g. both A roads, give lowest number.
    'Service Roads' should be classified as they appear (usually 'unclassified') and not as the class of the adjacent main road.

    Local Authority
    This may be completed by processing staff.
    Enter the code of the local authority in whose area the collision occurred.
    If in doubt about the code go to the Local Authority Codes list

    Location
    A minimum 10 figure Ordnance Survey Grid Reference is required.
    The Grid Reference should usually denote the first point of impact.
    ALWAYS! 'Easting then Northing' or 'Go across before going up'

    Police Force
    Enter the code of the police force in whose area the collision occurred
    It is important to ensure that where an accident is being reported that occurred in another police force area, a copy of the Stats 19 report is sent to that force.
    If in doubt about the code go to the Police Force Codes list.

    Number of Vehicles
    Enter the total number of vehicles involved in the collision. This should be the same as the number of  'vehicle records' included in the collision report.
    Always number from the right e.g. 002

    Enter the total number of casualties resulting from the collision. This should be the same as the number of  'casualty records'; included in the collision report.

    Road Type
    If the collision is on the footway/verge etc. select as per the road.
    Give general appearance e.g. single carriageway divided by a short island before a junction is still a single carriageway.
    Roundabouts include mini roundabouts and gyratory systems EXCEPT where the gyratory system is a system of local roads.
    Roads are only dual carriageways if they have a central reservation or barrier.
    One way streets with contra flow bus or cycle lanes will be treated as single or dual carriageways depending on whether the cycle or bus lane is separated from opposing traffic by a painted line (S/Carr) or a physical island or barrier (D/Carr)

    Speed Limit (Permanent)
    Give the general/normal Speed Limit for the road.
    Do not give temporary limits or vehicle limits.

    Junction Detail
    Using the list, choose the junction description that best describes the location of the collision.
    Treat Y junctions as T junctions.
    Private Roads which are in use, count as roads for the purpose of junction description.

    Junction Control
    Only complete if collision is at a junction.
    An 'authorised person' is any person with the power to stop and direct traffic i.e. police officer, school crossing patrol etc.
    If there is both an authorised person controlling the traffic and automatic traffic signals (ATS) then enter 'authorised person'. If the ATS are off and there is no authorised person, enter ATS and clarify under special conditions at site variable 1.24.

    Pedestrian Crossing - Human Control
    This ONLY applies to a 'human control' crossing where
    Either
    A pedestrian is involved in the collision on or within 50m of the crossing
    Or
    the crossing is within 50m of the collision location AND it was on the route of one of the vehicles involved AND it was considered to have been a factor in the collision (whether or not a pedestrian was involved)

    'Authorised person' means someone other than a school crossing patrol with legal authority to stop and direct traffic e.g. Police Officer, Traffic Warden.
    It makes no difference whether the 'human control' is operating on a 'physical crossing' e.g. School Crossing Patrol operating on a Zebra Crossing.

    Pedestrian Crossing - Physical Facilities
    This ONLY applies to 'physical pedestrian facilities' as listed where
    Either
    A pedestrian is involved in the collision on or within 50m of the crossing
    Or
    the crossing is within 50m of the collision location AND it was on the route of one of the vehicles involved AND it was considered to have been a factor in the collision (whether or not a pedestrian was directly involved)

    A Zebra Crossing with a defective beacon, stripes or zigzags is still a 'Zebra Crossing'.
    Raised 'speed tables' are not a crossing facility unless specifically marked.

    Weather
    Choose the code which most accurately describes the conditions.
    If two or more codes apply, choose the one which most adversely affected driving.

    Choose the code which most accurately describes the conditions.

    Light Conditions
    Darkness means half and hour after sunset until half an hour before sunrise.
    Where, during hours of darkness, some of the street lights are unlit, apply the following criteria;
  • Where alternate lamps are lit, code as 'street lights lit'

  • Where scattered or isolated lamps are lit, code as 'street lights unlit'

    Special Conditions at Site
    If any of the listed Special Conditions exist, they apply and should be recorded, regardless of whether they contributed to the collision.
  • ATS includes traffic signals at junctions and Puffin, Pelican and Toucan type crossings.

  • ATS out should be used where the ATS were totally inoperative. Only include part time ATS if they should have been in operation but were not.

  • If road works were present and were hit, code 'road works' even if other special conditions also exist. This includes collisions in the coned area before the actual works.

  • A road surface is defective if, for example, cracked, potholed or molten, and not if flooded or frozen.

    Carriageway Hazards
    'Other Object' relates to objects not expected to be found in the carriageway but does not include loads dislodged from vehicles involved in this collision.
    If 'involvement with previous accident' and another code both apply, code 'involvement with previous accident'
    'Animal' is not restricted to Road Traffic Act definition. NB Treat dead animals (and dead pedestrians) as 'objects in carriageway'.

    Did a Police Officer attend the scene and obtain the details for this report?
    This is to help indicate the reliability of the data. More reliance will be given if it is known that data was obtained by an officer while at the scene.

    To avoid duplication 'greyed out' fields do not have to be completed in all circumstances. Repeating this detail will only be necessary in those police forces which separate the two parts of the report and where steps have not been taken to have this 'duplication' dealt with as a 'back office' measure.

    Protective marking scheme
    The National Collision Report Forms are fully compliant with Protective Marking standards. The information on the pages of the report marked 'RESTRICTED' will only be shared with those having a right to the information. The statistical forms, which contain no personal data, are marked 'UNCLASSIFIED' and may be passed to others unrestricted.
     
    Each statistical 'field' is governed by rules published in STATS 20. The notes below are a paraphrase of some of these rules, included because of the frequency of enquiries about them. These bullet points are not a comprehensive list. In completing these statistical fields it MUST BE REMEMBERED that the code selected in one field may have a direct effect on codes in other fields. These cross-field relationships are fully explained in STATS 20 which should be referred to in any case of doubt.
