Direction of Vehicle Travel
Using the boxes provided, show, in the 'From' box, the direction from which the vehicle had come prior to the collision and, in the 'To' box, its intended direction had the collision not occurred. Stationary vehicles waiting to turn or move off should be treated as though moving. At roundabout, code for the action being undertaken e.g. turning left, going straight ahead, turning right. Casualties to be reported
Record any person(s) killed or injured in the collision.
Vehicle Reference Number
The vehicle reference of the vehicle occupied by the casualty number MUST be the same as the vehicle reference given to it in the police part of the Collision Report e.g. 001 or 002 etc. Pedestrian casualties will be allotted the reference number of the vehicle in collision with. Casualty Home Postcode
Complete the casualty home postcode in two parts as indicated. If there is no home postcode or it is not known, code; 1. Unknown 2. Non-UK resident Students living away from home should give the postcode for their temporary address during term time and for their permanent address at other times. Sex of Casualty

Age of Casualty
Where an exact age, in years, at last birthday is not known, enter an estimate. If no estimate is made it will not even be possible to classify the casualty as a child or adult. Therefore, every effort should be made to estimate an age. If an estimate is not possible, leave blank. Do not enter 00 or 99 to indicate not known. Severity of Casualty
The following definitions should be applied Fracture, Burns (excluding friction burns), Internal injury, Concussion, Severe cuts, Crushing, Severe general shock requiring hospital treatment, Detention in hospital as an in-patient, either immediately or later, Injuries to casualties who die 30 or more days after the accident from injuries sustained in that accident Sprains (not necessarily requiring medical treatment), Neck whiplash injury, Bruises, Slight cuts, Slight shock requiring roadside attention. (Persons who are merely shaken and who have no other injury should not be included unless they receive or appear to need medical treatment). School Pupil Casualty
A 'pupil' attending school includes any child up to and including 16 years of age and includes those below school age attending playgroups. A 'journey to or from school' includes journeys to or from pre-school or after-school activities based at the school but excludes journeys made to or from school activities which are not based at the school. Car Passenger
This relates only to passengers in cars or in taxis or private hire vehicles. Car drivers are NOT passengers. Passenger casualties riding on the outside of a vehicle will be regarded as front seat passengers. Click on field on form for example
Bus or Coach Passenger
A person injured in a collision after alighting from a bus etc. will be regarded as a pedestrian. Bus includes any PCV with a carrying capacity of 17 or more seats including the driver. Click on field on form for example
Pedestrian Location
To be completed for PEDESTRIAN Casualties only. A 'pedestrian crossing the road' is one wholly or partly on the carriageway who has the intention to cross from one footway to another, or to a central reservation or refuge. Click on field on form for example
Pedestrian Movement
To be completed for PEDESTRIAN Casualties only. 'Crossing from driver's side etc' should be interpreted as relating to the first vehicle which struck the pedestrian. A pedestrian 'in the carriageway' includes someone walking aimlessly or lying in the carriageway e.g. drunk. Click on field on form for example
Pedestrian Direction
To be completed for PEDESTRIAN Casualties only. Use the compass illustration as a guide and give the directions to the nearest numbered compass point. Click on field on form for example
Pedestrian injured in the course of 'on the road' work.
Includes those actively carrying out work on the public highway, such as delivery services, road maintenance, postal delivery, traffic control etc. It does not include a person who while 'at work' is merely travelling from place to place. Click on field on form for example To avoid duplication 'greyed out' fields do not have to be completed in all circumstances. Repeating this detail will only be necessary in those police forces which separate the two parts of the report and where steps have not been taken to have this 'duplication' dealt with as a 'back office' measure.

Protective marking scheme
The National Collision Report Forms are fully compliant with Protective Marking standards. The information on the pages of the report marked 'RESTRICTED' will only be shared with those having a right to the information. The statistical forms, which contain no personal data, are marked 'UNCLASSIFIED' and may be passed to others unrestricted. |
