Protective marking scheme
The National Collision Report Forms are fully compliant with Protective Marking standards. The information on the pages of the report marked 'RESTRICTED' will only be shared with those having a right to the information. The statistical forms, which contain no personal data, are marked 'UNCLASSIFIED' and may be passed to others unrestricted.
Sketch Plan
This is one of two possible sketch plan layouts that may appear in police collision report forms. The direction of North should be indicated in the place provided. The plan will usually be used to record the angle and position of the vehicles after coming to rest after the collision and not necessarily to record the path or intended destination of the vehicle or pedestrian - though this may be added if it will assist in understanding what has occurred. Identify the vehicles by both their registration mark and the vehicle number allocated in the report e.g. 001, 002, etc.
Measurements taken by
Use this space to record the identity of the person taking the measurements, and the time and date they were recorded.

Direction of vehicle travel
Using the boxes provided, show, in the 'From' box, the direction from which the vehicle had come prior to the collision and, in the 'To' box, its intended direction had the collision not occurred.
Stationary vehicles waiting to turn or move off should be treated as though moving. At roundabout, code for the action being undertaken e.g. turning left, going straight ahead, turning right.

Each 'field' is governed by rules. In completing these fields it MUST BE REMEMBERED that the codes selected in one field may have a direct effect on codes in other fields.
