Protective marking scheme
The National Collision Report Forms are fully compliant with Protective Marking standards. The information on the pages of the report marked 'RESTRICTED' will only be shared with those having a right to the information. The statistical forms, which contain no personal data, are marked 'UNCLASSIFIED' and may be passed to others unrestricted
Vehicle number
Record in this box the number of the vehicle being driven by the person being interviewed as recorded elsewhere in the report e.g. 001, 002 etc. Click on field on form for example
Record of Interview at the scene
Use this form to record any interview conducted at the scene. This form is available in two formats that deal with either one or two persons.
Personal details
Enter the person's family or surname and indicate their title e.g. Mr or Ms etc. Others titles could include 'Doctor', 'Professor' etc. Enter the person's forenames and date of birth. Enter the person's usual address (usually home address) and Post Code. Click on field on form for example

Use this part of the form to record the administration of the caution, the questions and answers shown and to record the persons account. The interview record should be signed by the person and countersigned by the interviewing police officer. Click on field on form for example
