Protective marking scheme
The National Collision Report Forms are fully compliant with Protective Marking standards. The information on the pages of the report marked 'RESTRICTED' will only be shared with those having a right to the information. The statistical forms, which contain no personal data, are marked 'UNCLASSIFIED' and may be passed to others unrestricted
Number of casualties
Enter the total number of casualties injured in the collision (including those whose details may have been recorded in another collision report(s)). Click on field on form for example
Number of vehicles
Enter the total number of vehicles involved in the collision (including those the details of which may have been recorded in another officer's report). Click on field on form for example
Number of reports
A collision reported using one set of forms by one officer would read: Report 01 of 01 Reports. Where a collision is reported using two or more sets of forms by one or more officers, The first form would read: Report 01 of 02 Reports, The second or subsequent reports could read: Report 02 of 02 Reports etc. Click on field on form for examples
Severity of Injury
One only of the definitions will be circled to describe the overall collision assessment relative to the most serious injury to any person involved in the collision as defined in the Stats 20 definitions below. (This does not relate to animal injuries) FATAL - Where death occurs in less than 30 days as a result of the collision and does not include death from natural causes or suicide. SERIOUS - Examples of a serious injury include: Fracture, Burns (excluding friction burns), Internal injury, Concussion, Severe cuts, Crushing, Severe general shock requiring hospital treatment, Detention in hospital as an in-patient, either immediately or later, Injuries to casualties who die 30 or more days after the accident from injuries sustained in that accident. SLIGHT - Examples of a slight injury include: Sprains, not necessarily requiring medical treatment, Neck whiplash injury, Bruises, Slight cuts, Slight shock requiring roadside attention (Persons who are merely shaken and who have no other injury should not be included unless they receive or appear to need medical treatment). Non-Stop
Place a cross in the adjacent box if any one or more vehicles involved in the collision has failed to stop or has stopped but failed to provide necessary details to anyone having reasonable grounds for requiring them. Click on field on form for example
Police Vehicle involved
Place a cross in the adjacent box if a police vehicle has been involved in or (following your police force policy) has been in the vicinity or contributory to the collision. Click on field on form for example
Time
Enter the time the collision occurred using the 24 hour clock. Enter 0001 if the collision occurred at midnight. Click on field on form for example
Day
Circle the day of the week on which the collision occurred. Click on field on form for example
Date
Enter the date on which the collision occurred. The form will have to be manually adapted if the collision occured between dates. Click on field on form for example
1st Road Class and Number
Enter Road Class and then Road Number e.g. A127 or B1066. If collision is at a junction give the road with priority (right of way). If roads have equal right of way give A road before B road etc. If roads are equal class e.g. both A roads, give lowest number. 'Service Roads' should be classified as they appear (usually 'unclassified') and not as the class of the adjacent main road. Enter UC if the road is unclassified. Enter NK if the classification is not known. Click on field on form for examples
1st Road Name and Number
Enter the name of the 'first road' (see first road class and number), or location. Click on field on form for example
Speed Limit
Give the speed limit in force on the first road at the time of the collision. Tick the adjacent 'temp' box if the speed limit in force at the time, is a temporary limit, e.g. road works. (NB. note that the permanent speed limit for the road is always required to be entered on the Stats 19 form.) Click on field on form for examples
Location
Completion of this field will depend upon the nature of location. Urban locations are often best described by giving an adjacent house number or building name. Motorway locations will make use of the number of the nearest marker post. In rural locations it may be necessary to leave this box uncompleted when the reporting officer can rely upon a distance from another road or location to best describe where the collision occurred. Location
Use these boxes to indicate the distance in metres NSEW of the second road or location. In rural locations this could be a considerable distance. Click on field on form for examples
2nd Road Class and Number
Enter Road Class and then Road Number e.g. A127 or B1066. If collision is at a junction give the 'minor' road. 'Service Roads' should be classified as they appear (usually 'unclassified') and not as the class of the adjacent main road. Enter UC if the road is unclassified. Parish Number and Name
Enter the Parish number or name, if known and if required by local directions. Click on field on form for examples
Sector/Beat Number
Enter the Sector or Beat Number as required by local directions. Click on field on form for example
Division
Enter the Division as required by local directions. Click on field on form for example
Grid Reference
Many police forces require this information to be left uncompleted by the reporting officer. In these cases the information will be determined by back-office staff. If the information is required: A minimum 10 figure Ordnance Survey Grid Reference should be entered. The Grid Reference should usually denote the first point of impact. Use the link in the example box to help determine grid reference. ALWAYS! 'Easting then Northing' or 'Go across, before going up'. Click on field on form for example and help
Time
Enter the time the collision was reported by police (not the time the collision was notified to police - this will be recorded elsewhere) Enter 0001 if the collision was reported by police at midnight. Click on field on form for example
Date
Enter the date the collision was reported by police (not the date the collision was notified to police - this will be recorded elsewhere) Click on field on form for example
Where reported
If the collision was reported at the scene of the collision, enter a If the collision was reported elsewhere, enter details of the location e.g. Nonesuch Police Station, Central Hospital, Home address of driver vehicle one etc. Click on field on form for examples
Person making report
Enter the name of the person making the report to police. The identity of this person will normally appear elsewhere in the report. Click on field on form for example
Reporting Officer
Enter the Name and Number of the police officer or member of police staff completing the report, the code of the BCU or division and station to which attached. Click on field on form for example
Time of arrival at the scene
Enter the time the reporting officer arrived at the scene of the collision. Leave blank if the collision was not recorded at the scene, even if the officer attended later for some purpose. Click on field on form for example
Photos / Video taken
Indicate whether photographs have been taken or a video recording made. Click on field on form for example
Collision Investigator attended
Indicate whether a Collision Investigator attended the scene for collision reconstruction purposes. Click on field on form for example
Investigating Officer
Enter the name, number and station or unit to which attached, of the officer deputed to investigate the collision, if different from the reporting officer. Some police forces may require these boxes to be left uncompleted. Click on field on form for example
How the collision occurred
Enter a description of how the collision occurred. It is appreciated that this will be completed at an early stage of the investigation and possibly before all witnesses or parties have been seen by police. Officers should not be concerned that the collision may be re-assessed when the full facts are known. This is an initial opinion only to aid those considering the implications of the collision. A copy of this part of the report is frequently sent to the Local Authority to aid validation of STATS 19 data and detailed analyses of individual collisions. Click on field on form for example
