Protective marking scheme
The National Collision Report Forms are fully compliant with Protective Marking standards. The information on the pages of the report marked 'RESTRICTED' will only be shared with those having a right to the information. The statistical forms, which contain no personal data, are marked 'UNCLASSIFIED' and may be passed to others unrestricted
Vehicle number
Record in this box a number to distinguish each vehicle involved in the collision. This number will be used to help describe how the collision occurred and will be used to denote which vehicle was occupied by which casualty etc, etc. For example, in a two vehicle collision, the vehicles will be numbered 001 and 002. Special categories
This information will help the reader of the report to quickly appreciate the significance of each vehicle's involvement in the collision. Check 'Non Stop (contributory)' if the vehicle was a contributory factor in the collision but was not directly involved by colliding with anything, and the vehicle did not stop. Check 'Hit and Run' if the vehicle was directly involved in the collision and then failed to stop and provide details to anyone having reasonable grounds for requiring them. Check 'Parked' if the vehicle was directly involved in (or contributory to) the collision and it was parked at the time. Click on field on form for examples
Driver and Vehicle Details
Use this page to record details of a vehicle and its driver. Pedal Cycles and Horses which are being ridden should be regarded as 'vehicles' and details entered on this page.
Personal details
Enter the driver's family or surname and indicate their title e.g. Mr or Ms etc. Others titles could include 'Doctor', 'Professor' etc. Enter the driver's forenames, date of birth and any other identification which might be relevant e.g. a taxi driver's badge number or the service number of military personnel. Enter the driver's usual address (usually home address) and Post Code. Enter contact email address and telephone numbers indicating D = daytime, E = evening, M = mobile. Ethnicity Codes
This information may be collected by police forces to aid the interpretation and analysis of collision data. These fields should be completed as required by individual police force policy. 16+1 ethnicity code: Invite the person to select from the list published on the Notes and Instructions Page (or a list published elsewhere) which he or she considers best describes them. This is not mandatory. Officer defined ID Code: The officer should choose one of the two digit codes, from the Officer defined Pheonix, list which best describes the person. Not Stated codes: In cases where the person declines to select a code from the 16+1 list, it is the policy of some police forces for the officer to enter, in the 16+1 code box, a code from the list of Not Stated codes. Registration Mark
Enter, in clear block capitals, the registration mark of the vehicle. Click on field on form for example
Vehicle details
Enter in these fields the make, model and colour of the vehicle. Circle YES or NO to indicate whether the road was a motorway, whether the vehicle was manual gear change, displaying L Plates, towing a trailer, carrying passengers and whether the driver was supervised by a full licence holder. These details will help determine the type of driving licence required. Click on field on form for example
Type of use
Indicate, by ticking the relevant box, whether the vehicle is being used for business purposes or for social, domestic and pleasure purposes. This information will help determine the type of vehicle insurance required. Click on field on form for example
Test Certificate (MOT)
Indicate, by circling YES or NO whether the MOT Test Certificate is correct. Click on field on form for example
Production of driving documents
Where driving documents have not been produced at the time of reporting the collision, and a form HORT1 is issued for their production at a nominated police station, the name of the police station nominated should be entered and the documents to be produced ticked by reference to the table at the foot of the form. Casualty Driver / Rider number
Where the driver or rider is injured, a casualty reference number must be allocated. For example, a collision involving two vehicles and an injured pedestrian could have two injured people, the pedestrian and the driver of one of the vehicles. The details of the pedestrian will be entered on the page dealing with casualties, while the details of the injured driver will remain self-contained on the page dealing with that driver and his vehicle. It is essential that each is allocated their own casualty reference number. In this case the injured driver would normally be allocated 001 and the injured pedestrian 002. Severity of injury
For each individual casualty there must be a classification of the severity of the injury. FATAL, SERIOUS or SLIGHT must be circled in accordance with the following definitions. FATAL - Where death occurs in less than 30 days as a result of the collision and does not include death from natural causes or suicide SERIOUS - Examples of a serious injury include: Fracture, Burns (excluding friction burns), Internal injury, Concussion, Severe cuts, Crushing, Severe general shock requiring hospital treatment, Detention in hospital as an in-patient; either immediately or later, Injuries to casualties who die 30 or more days after the accident from injuries sustained in that accident. SLIGHT - Examples of a slight injury include: Sprains; not necessarily requiring medical treatment, Neck whiplash injury, Bruises, Slight cuts, Slight shock requiring roadside attention. (Persons who are merely shaken and who have no other injury should not be included unless they receive or appear to need medical treatment). Click on field on form for example
Details of injury etc
This field will be used to record a brief description of injuries or genuinely suspected injuries. For example; Broken left leg, cuts to the face and possible chest injury. OR if more detail is available from the hospital, Broken lower left leg, facial lacerations, broken rib. If taken to hospital, enter the name of the hospital and indicate whether detained or not detained. A casualty will be considered to be 'detained' if detained in hospital for 24 hours or more. Indicate whether relatives of the injured person have been informed. If the casualty is of school age, indicate whether he or she is a pupil on a journey to or from school. The following criteria will help to decide if the pupil will be considered to be on such a journey; A 'pupil' attending school includes any child up to and including 16 years of age and includes those below school age attending playgroups. A 'journey to/from school' includes journeys to/from pre-school or after-school activities based at the school but excludes journeys made to/from school activities which are not based at the school. Vehicle Excise Licence (tax disc) irregularities
In cases where Vehicle Excise Licence irregularities have been detected, indicate whether forms CLE 2/6, 2/7 or 2/8 have been submitted. Click on field on form for example
HORT1 Code
This HORT1 Code grid is used to determine the 'number' of any documents to subsequently be produced at a police station. The 'number' will be entered on this page in the HORT1 field beneath the Vehicle Details.
Vehicle defect alleged
Indicate whether or not any vehicle defect has been alleged. Details of any vehicle examination or other action will be recorded elsewhere in the report. Click on field on form for example
Driver's initial comments
This field will be used to record any immediate initial comment made by the driver before any caution can be administered. Where a driver is cautioned and a further statement or comment recorded, this may be entered here (if there is sufficient remaining space) or elsewhere in the report (according to your force instructions) usually in the place provided for any Record of Interview at the Scene. Places are provided for recording the time of the comment and for the driver to sign if this is desired and possible. Click on field on form for example
Driving documents
If a Driving Licence is produced at the time of reporting, the sixteen digit driver number should be recorded together with an indication as to whether the licence was FULL or PROVISIONAL. Enter the name of the Insurance Company and the Certificate Number. Click on field on form for example
Impairment
Breath Tests Use this field to indicate either the time any screening (preliminary) breath test was administered and the result, including the alcohol concentration if known, or, if a breath test was not administered, the reason for not doing so. Impairment and Eyesight Tests Also indicate whether a Preliminary Impairment Test or Eyesight Test was conducted. The result will be recorded elsewhere. Click on field on form for example
Goods Vehicle Details
Where the vehicle is a goods vehicle, this field should be completed as appropriate. Click on field on form for example
Vehicle Damage
Use this field to record a description of the damage to the vehicle. Although great detail is not generally required, avoid generalisations like 'Dents and scratches all round'. Typical examples could be: FOS (front offside) wing and door dented, front bumper buckled, headlamp assembly smashed OR Extensive rear damage including boot lid, rear bumper and lamp assemblies. FNS (front nearside) wing dented, FNS wheel bucked and tyre deflated. Bonnet buckled. Windscreen cracked. (Nearside is the side of the car nearest to the left kerb when the vehicle is being driven. Offside is the other side) Use If the vehicle was removed under arrangements made by police, record the name of the person making the arrangements, and the place to where it was removed and by whom. Indicate whether or not the damage is so extreme that the vehicle may be a total loss. Click on field on form for examples
