Protective marking scheme
The National Collision Report Forms are fully compliant with Protective Marking standards. The information on the pages of the report marked 'RESTRICTED' will only be shared with those having a right to the information. The statistical forms, which contain no personal data, are marked 'UNCLASSIFIED' and may be passed to others unrestricted
Casualties, etc
This page is provided to record the details of injuries to casualties other than drivers, details of witnesses, owners of other property which may have been damaged and other persons somehow involved in the collision but not recorded elsewhere. Each page caters for two such people. Use each such column to record the details of one person and tick the boxes in the 'category' field to indicate whether the person is a casualty, a witness, or the owner of other property etc.
Casualties, etc
Complete Section A if the person is a witness or 'other person' involved in the collision, Complete Section A and Section B if the person is a casualty (other than a driver), Complete Section A and Section C if the person is the owner of other property damaged in the collision. Personal Details
Enter the person's family or surname and indicate their title e.g. Mr or Ms etc. Others titles could include 'Doctor', 'Professor' etc. Enter the person's forenames and date of birth. Enter the person's usual address (usually home address) and Post Code. Enter contact email address and telephone numbers indicating D = daytime, E = evening, M = mobile. Ethnicity Code
This information may be collected by police forces to aid the interpretation and analysis of collision data. These fields should be completed as required by individual police force policy. 16+1 ethnicity code: Invite the person to select from the list published on the Notes and Instructions Page (or a list published elsewhere) which he or she considers best describes them. This is not mandatory. Officer defined ID Code: The officer should choose one of the two digit codes, from the Officer defined Pheonix list, which best describes the person Not Stated codes: In cases where the person declines to select a code from the 16+1 list, it is the policy of some police forces for the officer to enter, in the 16+1 code box, a code from the list of Not Stated codes. Passenger position
Enter the vehicle in which the passenger was being carried e.g. 001, 002, 003 etc and indicate whether the passenger occupied a front or rear seat. Click on field on form for example
Pedestrian casualties
Where a pedestrian is a casualty, record the number of the vehicle that struck them first e.g. 001, 002, 003 etc Click on field on form for example
Witness independence
If the person whose details are being recorded is a witness, indicate whether or not they are considered to be 'independent'. It is highly unlikely that a witness who is a passenger in one of the vehicles involved (except perhaps a bus or taxi) could be considered 'independent'. Indicate whether the witness is willing to attend court should this be necessary. Indicate whether a written statement on Form MG11 has been taken. Click on field on form for example
Initial comments
Record any initial comments or account that the person may provide. This will be particularly useful in helping to determine how the collision occurred and whether the person should be seen again for the purpose of recording a written statement. Casualty number and severity
For each casualty a casualty reference number must be allocated. For example, a collision involving two vehicles and an injured pedestrian could have three injured people, the pedestrian, the driver of one of the vehicles and a passenger in one of the vehicles. The details of the pedestrian and the injured passenger will be entered as casualties on this page. The details of the injured driver will have been recorded on the page dealing with that driver and his vehicle. It is essential that each is allocated their own casualty reference number. In this case the injured driver would normally have been allocated 001 on the driver and vehicle page and the injured pedestrian and injured passenger, shown on this page, allocated 002 and 003. For each individual casualty there must be a classification of the severity of the injury. FATAL, SERIOUS or SLIGHT must be circled in accordance with the definitions found in Stats 20. Details of injury etc
This field will be used to record a brief description of injuries. For example; Broken left leg, cuts to the face and possible chest injury. OR if more detail is available from the hospital, Broken lower left leg, facial lacerations, broken rib. If taken to hospital, enter the name of the hospital and indicate whether detained or not detained. A casualty will be considered to be 'detained' if they have been admitted as an 'in patient'. Indicate whether relatives of the injured person have been informed. If the casualty is of school age, indicate whether he or she is a pupil on a journey to or from school. The following criteria will help to decide if the pupil will be considered to be on such a journey; A 'pupil' attending school includes any child up to and including 16 years of age and includes those below school age attending playgroups. A 'journey to/from school' includes journeys to/from pre-school or after-school activities based at the school but excludes journeys made to/from school activities which are not based at the school. Other property damaged
This field will be used to record details of other property damaged and whether or not the owner has been informed. This will not be used to record damage to vehicles, cycles or ridden horses or to record damage to clothing or personal effects in or on any of the vehicles etc, or belonging to other casualties. This field is primarily intended for the recording of damaged roadside property such as buildings, street furniture, walls and fences etc. The owner is often the local highway authority who will in most cases be informed by police.
